Our team will reach out to you if you have a critically low glucose (sugar) reading and assist you. If we can't reach you, we will notify your designated emergency contact or emergency services if needed.

Check Your Eligibility

Begin by speaking with an Enrollment Specialist. Confirm if you qualify for Remote Patient Monitoring based on your health condition and insurance.

Receive Your Glucose Monitor

If you don't have your CGM yet, ADS/USmed will ship your CGM to you. Once it arrives, your health coach will call to get your Dexcom or Libre login details to connect your device to our monitoring system.

With your CGM connected to our system, our healthcare team will monitor your blood sugar levels 24/7, providing immediate support and intervention for better health management and peace of mind.
